The Washington Post has an article listing reputable organizations you can donate to.  The article can be found by clicking here.  I donated to an organization called GlobalGiving after checking on their credentials.

You can also check to see if there are any Ukrainian churches in your area that are accepting donations.  For example, I donated to St. Mary's Protectress Ukrainian Catholic Church in Apopka Florida.  They're accepting donations for things like blankets, food, hygiene products, tents and candles,  and they'll be shipping everything to Poland and then on to those in need.  They also have an Amazon page where you can purchase medical supplies that will be sent to Ukrainians.  (Be sure to choose the church's address for delivery!  The default is your own address!)
